29 CFR 1910.147(c)(4)(ii)(C):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (c) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and a) On or about August 11, 2023, the documented machine-specific procedures for the High Speed Doors (Rytec/ Rite Hite), which identified electronic and mechanical energy sources, did not contain specific procedural steps for shutting down, isolating, blocking and securing the electric and mechanical energy sources. The procedure did not list steps to remove locks and return the equipment to service.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) On or about August 16, 2023, the employer did not ensure that a periodic inspection of the energy control procedures such as but not limited to the Rytech High-Speed Door, located in the s-curve near the freezer, had been completed at least annually.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Each employee was not required to: (1) prepare for shutdown, (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (4) affix lockout or tagout devices to each energy-isolating device, (5) ensure that all potentially hazardous stored or residual energy is relieved, disconnected, restrained, and otherwise rendered safe, (6) verify the effectiveness of energy isolation. a) On or about August 11, 2023, the employer did not ensure that (d)(1) through (d)(6) was followed by authorized employees performing maintenance work on the Rytech high speed door by securing the electrical and mechanical energy sources for the door prior to starting work. Employees were exposed to caught-in hazards associated with the movement of the drive chain assembly, chain and sprocket when the door was activated.
