29 CFR  1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:  a) Worksite located at 2 Applegate Drive, Robbinsville Twp, NJ 08691:  Managers, Supervisors, Foremen and Leads applied locks to machines while other employees serviced and cleaned those machines. The locks were not applied by the employees who were actually servicing and maintaining the machines. Violation occurred on or about August 20,2023.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a. Worksite was located at 2 Applegate Drive, Robbinsville Twp, NJ 08691: Management did not ensure that lock out tag out procedures were developed for each machine and the lock out tag out procedures needed for the cleaning and servicing process. Procedures were not clear when workers cleaned machines including, but not limited to, the Taeheung Engineering Bag Dump Machine, DHW-32 Mini Wonton Machines and the Gyoza Dumpling Machines. Violation occurred on or about August 20, 2023.  b. Worksite was located at 2 Applegate Drive, Robbinsville Twp, NJ 08691: Workers tasked with working on Bag Dump machines were not the workers locking the machines out nor did they have locks to use as part of a group lockout to secure these machines during the deep cleaning process. Violation occurred on or about August 20, 2023.  The CJ TMI Manufacturing America, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1568863, citation number 2, item number 001 and was affirmed as a final order on 7/5/2022, with respect to a workplace located at 2 Applegate Drive, Robbinsville Twp, NJ 08691.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. The employee's right middle finger was partially amputated by the operating part of the machine. 29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) Worksite was located at 2 Applegate Drive, Robbinsville Twp, NJ 08691:  Management did not provide adequate training to ensure that supervisors and workers understood the requirements of lockout tagout as it pertained to the production line machines for servicing and maintenance. Workers were misidentified under the standard and not properly trained to be the ones performing the lockout tagout work during third shift daily and weekly cleaning processes. Supervisors were misidentified and did not understand their roles and were not performing the servicing and maintenance work on the machines they were tasked with locking  and tagging out. Machines requiring lockout included, but were not limited to, the Taeheung Engineering Bag Dump Machine, Taeheung Dough Press Rollers, DHW-32N2 Dumpling Formers, DHW-32 Mini Wonton Machines, Taeheung Oven Conveyor, Incline Conveyors, Conveyors and the Gyoza Dumpling Machines.  Violation occurred on or about August 20, 2023  The CJ TMI Manufacturing America, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(i), which was contained in OSHA inspection number 1405047, citation number 1, item number 002 and was affirmed as a final order on 11/26/2019, with respect to a workplace located at 2 Applegate Drive, Robbinsville Twp, NJ 08691.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
