29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about August 16, 2023, the employer failed to ensure the developed and documented procedures were utilized for the control of hazardous electrical, hydraulic and kinetic energy sources of puller #1 and puller #2, located in plant 2 at press #5, while employees were engaged in clearing extruded material that had got caught in puller #1 located in the puller runway area.

29 CFR 1910.147(c)(6)(i)(C):Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about On or about August 16, 2023, the employer failed to ensure the cooling table, located in plant 2 at press #5, was effectively guarded to prevent employees from jumping over the cooling table and entering the puller runway area.
