29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the commercial worksite located at 2300 Main Street, Kansas City Missouri 64108, on or about September 5, 2023: Employees were exposed to electrical shock, electrocution, and burns while using a frayed extension cord to plug in the portable generator. The extension cord had wires exposed at the end connector and was missing the insulation and guarding that protects the wires. The second extension cord had yellow tape around the area of the strain relief because it was also damaged.
