1926.21(b)(2) The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: At the temporary jobsite located at 9200 Cody Street Overland Park, Kansas, employees conducting roofing activities were exposed to fall hazards of approximately 30 ft. Employees were working around unprotected skylight measuring approximately 4ft by 4ft resulting in employee falling through the uncovered skylight approximately 30ft to the concrete floor below.
