29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: a) 2507 Boulevard Ave, Scranton, PA: Qualcraft roof anchors used with a fall arrest system were not installed in accordance with the manufacturer's instructions, which exposed employees to the potential for injury during a fall if the anchors were to fail, on or about September 28, 2023.
