29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  (a)  On or about October 3, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, employees performing decking work on the East side of the house were exposed to an approximate fall hazard of eight (8) feet while working from an unsecured platform supported by the forks of a rough terrain forklift (JLG SKYTRAK rough terrain forklifts (Model numbers: 10054)).  (b)  On or about October 3, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, employees performing decking work on the West side of the house were exposed to an approximate fall hazard of eight (8) feet while working from an unsecured platform supported by the forks of a rough terrain forklift (JLG SKYTRAK rough terrain forklifts (Model numbers: 10054)).

29 CFR  1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  (a) On or about October 3, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, an employee was observed working from a  stack of 4 foot by 8 foot sheets of 3/4 inch OSB sheathing supported on the forks of a JLG SKYTRAK rough terrain forklift (Model numbers: 10054), and elevated to a height of approximately eight (8) feet above the ground level on the east side of the building. The employee was installing decking on the roof of a new residential building without fall protection, exposing the employee to a fall hazard of approximately eight (8) feet to the ground below.  (b)On or about October 3, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, an employee was observed working from a  stack of 4 foot by 8 foot sheets of 3/4 inch OSB sheathing supported on the forks of a JLG SKYTRAK rough terrain forklift (Model numbers: 10054), and elevated to a height of approximately eight (8) feet above the ground level on the west side of the building. 29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about October 16, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, the employer did not block windows on the west side and north side of new residential home which exposed employees to a fall hazard of approximately eight (8) feet from window to the bottom of basement window wells.  (b) On or about October 16, 2023,  at the job site located at 1552 Lookout Court, Hartland, WI 53029, employees conducting roofing work on 5:12 pitch roof, on the west side of a new residential home were not wearing fall protection. This exposed employees to a fall hazard of approximately 12 feet from roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about October 3, 2023, at the job site located at 1552 Lookout Court, Hartland, WI 53029, an employee installing decking on a new residential home, used a Keller 12-foot Aluminum ladder  to access the roof, was not erected three (3) feet above the upper landing. The employee used the ladder to egress the roof when the JLG SKYTRAK rough terrain forklift failed in the up position. This exposed employees to a fall hazard of approximately 12 feet to the ground below.
