29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation  (a) On or about November 29, 2023, at a jobsite located at 1525 Sandhill Boulevard, Hartland, WI 53209, employees were operating pneumatic nail guns without eye protection while installing new decking on the roof of a new residential home. This exposed employees to a struck by hazard and eye injury.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about November 29, 2023,  at a jobsite located at 1525 Sandhill Boulevard, Hartland, WI 53209, employees conducting decking of a 10:12 pitch roof on the west side of an new residential home were not wearing fall protection. This exposed employees to a fall hazard of approximately 17 feet from roof to the ground below.  (b) On or about November 29, 2023, at a jobsite located at 1525 Sandhill Boulevard, Hartland, WI 53209, the employer did not block windows on the West and North side of a  new residential home while conducting interior carpentry work, exposing employees to a fall hazard of approximately 8 feet from the window to the ground.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about November 29, 2023, at a jobsite located at 1525 Sandhill Boulevard, Hartland, WI 53209, an employee installing roof decking on a new residential home, used an Keller 12-foot Aluminum ladder  to access the roof that was not erected three (3) feet above the upper landing.
