29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  (a) On or about February 21, 2024, at a jobsite located at 1380 Prairie Creek Circle, Oconomowoc, WI 53066, the employer failed to ensure that an employee tasked with operating an all-terrain powered industrial vehicle (PIV) (LG SKYTRAK rough terrain forklift Model: 10054) had received the required training therefor exposing employees to struck by hazards.

(a) On or about February 21, 2024, at a jobsite located at 1380 Prairie Creek Circle, Oconomowoc, WI 53066, the employer did not ensure that all employees that work on the JLG SKYTRAK rough terrain forklift Model: 10054  were trained to recognize hazards associated with working on scaffolding, therefor exposing employees to electrical hazards, fall hazards and falling object hazards.

This exposed employees to a struck by hazard and eye injury.
