29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Bowling Center:  Employees performed cleaning/janitorial tasks, such as degreasing, disinfecting, and sanitizing throughout the facility.  Corrosive products, such as but not limited to GS Neutral Disinfectant and Peroxide Multi-Surface Cleaner/Disinfectant, were used by employees without eye protection.  Employees were exposed to eye irritation and/or chemical burns.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Bowling Center:  Employees performed cleaning tasks, such as degreasing, disinfecting, and sanitizing.  Corrosive products, such as but not limited to GS Neutral Disinfectant and Peroxide Multi-Surface Cleaner/Disinfectant, were used by employees without adequate flushing equipment available in a nearby proximity.  Employees were exposed to skin and eye irritation and/or chemical burns.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  In the following areas, secondary containers of chemicals were missing hazard information descriptions: a) Pin setter area:  A secondary container of Lucas High Performance Heavy Duty Gear Oil was labeled "1183."  b) Pin setter area:  Two grease guns were observed without any identifiers. c) Pin setter area:  An open-lid container of blue grease was observed without any identifiers.  d) Maintenance office:  Two spray bottles were labeled "8409."  e) Janitor closet:  A spray bottle of GS Neutral Disinfectant Cleaner was missing general information on the chemical hazards.  f) Janitor closet:  A spray bottle of "3020" kitchen degreaser was in a bottle labeled Peroxide Multi-Surface Cleaner and Disinfectant. Employees were exposed to skin irritation hazards.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Bowling Center:  Employees performed cleaning operations with chemicals, such as but not limited to disinfectants and degreasers, without training on the hazard communication program upon initial assignment.   Employees were exposed to eye and skin irritant hazards.
