29 CFR 1910.212(b): Machines designed for a fixed location were not securely anchored to prevent walking or moving. On or about October 5, 2023, in the card destruction room of the Hard Rock Hotel and Casino in Wheatland, California, a standing drill press was found unanchored. Employees were exposed to crushing injuries from falling machinery.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling. In the maintenance shop, a mini refrigerator was plugged into a relocatable power tap. Employees were exposed to smoke inhalation and burn injuries.
