On October 6, 2023, employees of Daniel Guzman, dba: Daniel Guzman were performing a re-roof on a residential building located at 1242 Peruque Ridge Drive in Wentzville, Missouri. Employees cleaning up old roofing materials from around the residence did not don protective helmets when exposed to falling objects from above.

A ladder was placed on the front of the residence for access and egress to the roof. The side rails of the ladder did not extend 3 feet above the upper surface. The employees when using the ladder were exposed to fall hazards.
