29 CFR  1910.215(a)(4):On offhand grinding machines, a work rest was not available to be used to support the work and the work rest available was not adjusted closely to the wheel with a maximum opening of one-eighth inch.  (a) at 47201 Highway 62, Prospect, Oregon 97536, Crew Cache Building, on or about October 11, 2023 and at times prior thereto: Employees were exposed to caught in hazard from disc/wheel on the abrasive bench grinder with a work rest that was more than 1/8" away from the grinding surface.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.215 (a) (4), which was contained in OSHA inspection number 1345465, notice number 1, item number 2 and was affirmed as a final order on 02/08/2019, with respect to a workplace located at Forest Service, Middle Fork Ranger District, 46375 Hwy 58, West Fir, Oregon, 97492.

29 CFR 1910.151(c): When the eyes or body of any person were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a)   at 47201 Highway 62, Prospect, Oregon 97536, Hot Shot build, tool/laundry, on or about October 11, 2023, and at times prior thereto:  Employees were exposed to serious eye injuries from unserviceable eyewash  located in Kitchen sink area, for First-Aid use in the event of a splash and emergency exposure to an irritant chemical to the eyes, which could be aggravate the employee's condition, the eye wash was disassembled and could not be a source of a potential eye flush resulting in serious eye infection or irritation.               Condition:  Suitable facilities for quick drenching or flushing of the eyes were not provided in the immediate work area for emergency use for employees using (chemical), a corrosive. Simple green pro3 antibacterial, - Category 1; Skin Corrosive/Irritant - Category 2; Acute Toxicity - Oral 4; Acute Toxicity - Dermal 5) under 2012 OSHA Hazard Communication Standards (29 CFR 1910.1200)
