29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  (a) at 800 Lauriel Avenue, Butte Falls, Oregon, Fire Cache building, on or about October 11, 2023, and at times prior thereto: Employees were exposed to a fall hazard from an unprotected edge of a loft area of the fire cache building that was approximately 8' 6 5/8", resulting in serious injuries and disability to body and head.

29 CFR  1910.157(e)(2):Portable fire extinguishers were not visually inspected at least monthly:  (a) at  800 Lauriel Avenue, Butte Falls, Oregon , Fire Cache building, on or about October 11, 2023 and at times prior thereto: The employer did not ensure that fire extinguishers in the building were inspected monthly. This exposed employees to smoke inhalation hazards from potential delays from having a fire extinguisher not working.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1594183, notice number 1, item number 1a and was affirmed as a final order on8/18/2022, with respect to a workplace located at Forest Service, Parkdale Work Center.

(a) at  800 Lauriel Avenue, Butte Falls, Oregon , Recreation building, on or about October 11, 2023 and at times prior thereto:  Employees were exposed to fall/trip hazards  from exit route emergency light not operating properly which would impair employee vision in the event of an emergency, resulting in injuries, bruises, and minor disabilities.

Without an operating fire extinguisher, employees are exposed to fire and smoke inhalation hazards during an emergency.

29 CFR  1910.303(b)(2):Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.  (a) at  800 Lauriel Avenue, Butte Falls, Oregon , Recreation building, on or about October 11, 2023 and at times prior thereto:  Employees were exposed to fire and electrical shock  hazardous condition from a Relocatable Power Tap plugged into another Relocatable Power Tap located second floor, room office area, resulting in minor burns and electrical shock.  For Reference:  Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.
