29 CFR  1910.212(b):Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  (a) at 42861 Highway 242, Powers, Oregon , Engine Bay/ Fleet Maintenance building, on or about November 28, 2023 and at times prior thereto: Employees were exposed to pinch by and struck by hazard from rotating machinery parts of a fixed bench grinder not mounted to table top which would allow grinder to walk and move, resulting in extremities getting seriously injured or disabled.

29 CFR  1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  (a) at 42861 Highway 242, Powers, Oregon , Engine Bay/ Fleet Maintenance building, on or about November 28, 2023 and at times prior thereto: Employees were exposed to pinch by and struck by hazard from rotating machinery parts of a table mounted fixed abrasive grinder wheel  missing side guards, resulting in extremities getting seriously injured or disabled.

29 CFR  1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  (a) at 42861 Highway 242, Powers, Oregon , Engine Bay/ Fleet Maintenance building, on or about November 28, 2023 and at times prior thereto: Employees were exposed to struck by, pinch point, and contact with an exposed grinding disc/wheel in that the bench grinder did not have a tongue guard or a work rest on one side resulting in potentially serious body extremity injuries and disability from shrapnel and wheel fragments .  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.215 (b) (9), which was contained in OSHA inspection number 1345463, notice number 2, item number 1 and was affirmed as a final order on 02/08/2019, with respect to a workplace located at Forest Service, Middle Fork Ranger District, 46375 Hwy 58, West Fir, Oregon, 97492.

(a) at 42861 Highway 242, Powers, Oregon , outdoor storage fuel tanks, on or about November 28, 2023 and at times prior thereto: Employees were exposed to unknown chemical hazards from unlabeled containers or labels that were not legible.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.1200 (f) (10), which was contained in OSHA inspection number1685080, notice number 1, item number 3 and was affirmed as a final order on10/25/2023, with respect to a workplace located at Forest Service, Walla Walla Ranger District, 1415 West Rose Street, Walla Walla, Washington 99362.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.1200 (f) (10), which was contained in OSHA inspection number1686630, notice number 1, item number 1 and was affirmed as a final order on10/27/2023, with respect to a workplace located at Forest Service, Heppner Ranger District, 117 South Main Street, Heppner, Oregon 97836.

(a) at 42861 Highway 242, Powers, Oregon , Office building, on or about November 28, 2023 and at times prior thereto: employees were exposed to fall/trip hazards  from exit route emergency light not operating properly which would impair employee vision in the event of an emergency, resulting in injuries, bruises, and minor disabilities.
