Oxygen cylinders in storage were not separated from fuel-gas cylinders with a minimum distance of 20 feet or by a noncombustible barrier in several instances, including but not limited to: a) A gas cylinder of oxygen was stored next to a gas tank of acetylene on the torch without a noncombustible barrier. b) A gas cylinder of acetylene was stored among the empty section in the gas cylinder storage area of the welding shop/fabrication area. c) A gas cylinder of acetylene was stored next to LP tanks in the gas cylinder storage area of the welding shop/fabrication area.
