OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to caught between and struck by hazards due to unsafe operation of heavy equipment utilized to maneuver and manipulate logs: a.) Bradford Forest, Inc.; log yard: On or about August 22, 2023 and times prior thereto: An employee was operating a Prentice log grappler without adequate knowledge to operate the equipment. The machinery tipped over with the operator inside the cab due to boom placement. b.) Bradford Forest, Inc.; log yard: On or about October 11, 2023 and times prior thereto: Employees were operating the CAT Front End loader with visual distortions in the cab glass. The front windshield of the cab had a circular spider webbed crack that radiated outward through the front windshield. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
