29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  On or about October 18, 2023, the employer did not ensure employees were protected by a protective helmet.  Employees were exposed to a possible danger of head injury from impact or from falling or flying objects, while working below employees installing roofing material from the second story roof of an existing residential property.

(a)  On or about October 18, 2023, the employer did not ensure that employees using pneumatic nail guns were wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about October 18, 2023, employees conducting roofing work on an existing residential roof with an 8:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet over the back of the house and nine (9) feet over the front of the residential property.

This exposed employees to a fall hazard of approximately 18 feet over the back of the house and nine (9) feet over the front of the residential property.
