29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about July 10, 2024, located at 3370 E Gracie Lane, Oak Creek, WI 53154, the employer failed to ensure the extension ladder placed on the garage roof used to access the upper landing extended at least three feet, exposing employees to a fall hazard of approximately 8.5 feet to the lower garage roof and an additional 9 foot fall hazard to the ground.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s).  (a) On or about July 10, 2024, located at 3370 E Gracie Lane, Oak Creek, WI 53154,  the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected.  All Covered Exteriors LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 1 and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI 53149.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about July 10, 2024, located at 3370 E Gracie Lane, Oak Creek, WI 53154, the employer failed to ensure employees tasked with installing underlayment on a residential roof were protected from falling to the ground below, exposing the employees to a fall hazard of approximately 17.5 feet.  (b) On or about July 10, 2024, located at 3370 E Gracie Lane, Oak Creek, WI 53154, the employer failed to ensure employees tasked with performing tear-off operations on a residential roof were protected from falling to the ground below, exposing the employees to a fall hazard of approximately nine feet (9).  All Covered Exteriors LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 4, group letter a, and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI 53149.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a) On or about July 10, 2024, located at 3370 E Gracie Lane, Oak Creek, WI 53154, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall hazard of approximately 17.5 feet on the upper section of the roof and approximately 9 feet while working on the garage roof.  All Covered Exteriors LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 4, group letter b, and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI 53149.
