(b) Citation 1, Item 2, a repeat-serious violation of 19 CFR 1926.102(a)(1) with an abatement date of September 18, 2024. (c) Citation 1, Item 3, a repeat-serious violation of 19 CFR 1926 501(b)(13) with an abatement date of September 18, 2024. (d) Citation 1, Item 4, a repeat-serious violation of 19 CFR 1926.503(a)(1) with an abatement date of September 18, 2024. 29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s).  (a) On or about July 30, 2024, the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected. Hazards identified included but were not limited to: Fall protection not being worn and employees not provided with eye protection while using pneumatic tools.  All Covered Exteriors, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 1 and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  (a) On or about July 30, 2024, the employer did not ensure employees were protected from flying objects by providing safety glasses. Employees were exposed to a possible danger of eye injury from flying objects, while using a pneumatic nail gun to install drip edges on a two-story residential property.  All Covered Exteriors, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 3 and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about July 30, 2024, employees conducting roofing work on an existing residential roof with a 8:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18.5 feet from the upper two-story roof and 10 feet over the lower garage roof.  All Covered Exteriors, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1704302, citation number 1, item number 4, group letter a, and was affirmed as a final order on January 26, 2024, with respect to a workplace located at 904 Pinehurst Drive, Mukwonago, WI.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about July 30, 2024, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall hazard of approximately 18.5 feet from the upper two-story roof and 10 feet from the lower roof above the garage of the residential property.
