(b) On or about October 17, 2023 at the jobsite located at 519 South Street Oconomowoc, WI 53066, an employee was performing roofing work on a the first story of a residential building without the use of fall protection, therefore exposing employees to a fall hazard of approximately 11.3 feet.  JA Homeworx LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1658290, citation number 1, item number 1 and was affirmed as a final order on 4/18/23, with respect to a workplace located at 849 W. Wisconsin Ave, Oconomowoc WI 53066.
