29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. (a) On or about August 28, 2025, the portable ladder being used by an employee to access the 2nd floor balcony did not extend at least three feet above the upper point of contact with the Balcony, nor was the ladder secured in place. This exposed employees to a fall hazard of 15 feet onto grass and asphalt.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. (a) On or about August 28, 2025, an employee conducting balcony reconstruction activities to include decking the second floor balcony was not protected from falling to the lower level. This exposed the employee to a fall hazard of approximately 15 feet. JA Homeworx, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1704719, citation number 2, item number 1 and was affirmed as a final order on February 5, 2024, with respect to a workplace located at 519 South Street, Oconomowoc, WI 53066. JA Homeworx, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1658290 citation number 1, item number 1 and was affirmed as a final order on April 18, 2023, with respect to a workplace located at 849 W. Wisconsin Ave, Oconomowoc, WI 53066.
