29 CFR 1910.94(a)(3)(i)(e)(1): Doors were not flanged and tight when closed. On or about October 19, 2023 and at times prior thereto, the doors to the abrasive blasting machine was not flanged or closed tightly to prevent forge steel shot from leaking out of the machine. This exposed employees to a slip hazard.

29 CFR 1910.94(a)(4)(i)(a): When dust leaks were noted, repairs were not made as soon as possible. On or about October 19, 2023 and at times prior thereto, repairs were not made when steel shot was leaking out of the abrasive blasting machine and its duct work. This exposed the employees to slip hazards.

29 CFR 1910.94(a)(7): Dust was permitted to accumulate on the floor or on ledges outside of an abrasive-blasting enclosure, and dust spills were not cleaned up promptly. Aisles and walkways were not kept clear of steel shot or similar abrasive which may have created a slipping hazard. On or about October 19, 2023, and at times prior thereto, Steel forged shot was on the floor in two instances: a. On or about October 19, 2023, and at times prior thereto, steel forge shot was accumulated on top of the abrasive blasting machine and on the walking/working surface around the abrasive blasting machine. This exposed employees to a slip hazard. b. On or about October 19, 2023, and at times prior thereto, near the Hoffman Blast Room, steel forge shot was accumulated on the walking/working surface around the Pangborn dust collector. This exposed employees to a slip hazard.
