On or about October 27, 2023, Henry Uribe Romero DBA M A Uribe Roofing LLC did not ensure that a portable ladder used to access the roof of a residential home at 156 Langtree Drive in Wentzville, Missouri, was extended at least 3 feet above the roof. Employees were exposed to falls from heights up to approximately 15 feet.

On or about October 27, 2023, Henry Uribe Romero DBA M A Uribe Roofing LLC did not ensure that each employee roofing a residential home at 156 Langtree Drive in Wentzville, Missouri, was protected by personal fall arrest systems. Employees were exposed to falls from heights greater than 15 feet. M A Uribe Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1498098, citation number 1, item number 1 and was affirmed as a final order on February 1, 2021, with respect to a workplace located at Lot 49 Rhythm Point Drive in St. Peters, Missouri.

29 CFR 1926.59: The employer had not developed a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) as incorporated by reference: 29 CFR 1910.1200(e)(1): Employers had not developed, implemented, and maintained at each workplace, a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets (SDSs), and employee information and training would be met. On or about October 27, 2023, Henry Uribe Romero DBA M A Uribe Roofing LLC did not develop, implement, and maintain at each workplace, a written hazard communication program which at least described how the criteria in paragraphs (f) for labeling and other forms of warning, (g) for safety data Sheets (SDSs), and (h) for employee information and training would be met. Gas was utilized in a worksite at 156 Langtree Drive in Wentzville, Missouri, to power air compressors for the use of the pneumatic nail guns.
