Parts Room: On and/or before November 1, 2023, two Werner 4-foot A-frame ladders were used for maintenance to access elevated positions. The A-frame ladders used did not have any manufacture data listed for safety requirements nor load limits for employees to identify during inspection and use of ladder. Employees were exposed to the potential of ladder overloading, mechanical failure and/or fall hazard(s).
