29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a)	Framing and Finishing Services, LLC. On or about November 2, 2023, at the address of 5123 Main Street. Sylvania OH 43560: The employer failed to instruct employees in the recognition and avoidance of hazards commonly seen in the construction industry. Employee(s) performing framing activities without the benefit of effective training were exposed to fall hazards, struck-by hazards along with crushed beneath hazards.

On or about November 2, 2023, at the address of 5123 Main Street. Sylvania OH 43560: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns during framing work activities on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  a)	Framing and Finishing Services, LLC. On or about November 2, 2023, at the address of 5123 Main Street. Sylvania OH 43560: An employee performing residential sheathing work was using the sheathing material (OSB plywood) stacked on the forks of rough terrain forklift (serial# 0160007560, Model# 6036) as a temporary work platform without ensuring the platform was appropriately attached to the forks. The employee was exposed to potential fall hazards of approximately 6 feet to the ground below.
