At the front right side of the Garage of a temporary worksite located at 9532 Maidstone Mill Drive West, Jacksonville, Florida 32244: On or about November 8, 2023, the employer exposed employees to fall hazards in that, the side rails of a portable ladder used to gain access to the roof, did not extend three feet above the roof, exposing employees to a fall hazard.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. At a temporary worksite located at 9532 Maidstone Mill Drive West, Jacksonville, Florida 32244: On or about November 8, 2023, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down roof shingles.  Massey Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1607407, citation number 1, item number 1 and was affirmed as a final order on December 2, 2022, with respect to a workplace located at 5737 Earl Circle North, Jacksonville, FL 32219.

On a 7:12 pitch roof of a temporary worksite located at 9532 Maidstone Mill Drive West, Jacksonville, Florida 32244: On or about November 8, 2023, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while performing roofing work.  Massey Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1607407, citation number 1, item number 2 and was affirmed as a final order on December 2, 2022, with respect to a workplace located at 5737 Earl Circle North, Jacksonville, FL 32219.
