29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 2:12 pitch roof of a temporary worksite located at 4455 Water Oak Lane, Jacksonville, Florida 32244: On or about May 23, 2024, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while performing roofing work.  Massey Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1607407, citation number 1, item number 2 and was affirmed as a final order on December 2, 2022, with respect to a workplace located at 5737 Earl Circle North, Jacksonville, FL 32219.  Massey Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1709507, citation number 02, item number 002 and was affirmed as a final order on May 28, 2024, with respect to a workplace located at 9532 Maidstone Mill Drive West, Jacksonville, FL 32244.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. At the front right side of the Garage of a temporary worksite located at 4455 Water Oak Lane, Jacksonville, Florida 32244: On or about May 23, 2024, the employer exposed employees to fall hazards in that, the side rails of a portable ladder used to gain access to the roof, did not extend three feet above the roof, exposing employees to a fall hazard.
