29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) On or before 11/9/2023, the employer did not ensure that employees were protected from trench cave-in hazards. Employees were laying new water pipe in a 3' wide, 4'6" to 5'6" deep, and over 50' long trench. The soil was type B.
