Canton Ohio 44721, 8.4 feet above the ground were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure. Velazquez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1659486, citation number 2, item number 1 and was affirmed as a final order on May 4, 2023, with respect to a workplace located at 4544 Fenn Rd, Medina Ohio 44215. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about November 20, 2023, employees were exposed to a fall of approximately 8.4 feet while performing roofing activities on a residential home located at 1431 Schneider ST NE, Canton Ohio 44721. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area. Velazquez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053 (b)(1) Ladders, which was contained in OSHA inspection number 1567781, citation number 1, item number 2 and was affirmed as a final order on January 19, 2022, with respect to a workplace located at 3014 Copley Road, Copley, OH 44321.
