29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Rear roof: On or about October 6, 2025, employees performing roofing work were exposed to falls of 23 feet on a low slope roof due to a lack of any fall protection system being utilized. Abatement certification and documentation required.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Rear roof: On or about October 6, 2025, employees were exposed to fall hazards in excess of twenty-three (23) feet when entering and exiting the rear roof due to a portable ladder that did not extend at least three (3) feet above the roof's surface. Abatement certification required.
