29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a- Building 5, Lumberton NJ:   A worker applying sheathing onto the exterior surface of a commercial structure's pitched roof trusses, and performing other related activities, was not protected from falling while wearing a full body harness that was not attached to an anchor with a lanyard or other approved means.  As such, this worker was exposed to fall hazards in excess of 30 feet 4 inches to the ground surface below. Condition observed and noted on or about 12/05/23.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:   a- Building 5, Lumberton NJ:   Workers applying sheathing onto the exterior surface of a commercial structure's pitched roof trusses, and performing other related activities, were not protected from falling while wearing full body harnesses whose self retracting life line's housing were attached to nylon straps that were wrapped around either a single wooden roof truss chord or metal gusset plate within a roof truss.   As such, these workers were exposed to fall hazards in excess of 30 feet 4 inches to the ground surface below. Condition observed and noted on or about 12/05/23.  b - Building 5, Lumberton NJ:   A worker, walking within the inner chords of wooden roof trusses, was not protected to fall hazards when their self retracting life line's extended line was exposed to being damaged and would not have retracted as designed if the worker had fallen while the worker was within these trusses .  As such, this worker was exposed to fall hazards in excess of 10 feet 8 inches to the flooring below. Condition observed and noted on or about 12/05/23.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a - Building 5, Lumberton NJ:   Workers sheathing the pitched roof of a commercial building were not trained on fall protection means and methods as specified in 29 CFR 1926.503(a)(2)(i) through (viii).  These workers were exposed to fall hazards up to 30 feet 4 inches while wearing full body harnesses whose attached self retracting life line's housing were anchored improperly and not being used as per the manufacturer's requirements. Condition noted on or about 12/05/23.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
