29 CFR  1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:  On the lower deck of the ship Balclutha, there were large gaps in the floor planking and uneven walking/working surfaces. One section of the walking working surface had exposed stone with a single plank across it. Employees were exposed to heights and potential falls into the gaps.

29 CFR  1910.184(c)(6): Slings were not securely attached to their loads:  Outside of the small crafts work shop on the Hyde St. Pier, a small craft was being held up by slings. A purple synthetic web sling was affixed to a rod at the bow in a way that it could slip off of the rod if the load shifted or was moved. Employees are exposed to potential falling loads.

29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  Museum Building Boiler room: The machine guard over a rotating belt connected to a NIDEC motor, catalog # D5P20, did not fully enclose the in-going nip points or the end of the rotating belt.   Exhibits Room: A green Woodtek drill press did not have a guard at the point of operation (chuck).  BMU Facilities: 1). A green Wells Index milling machine did not have a guard at the point of operation.  2). A Grizzly Industries model G0709 lathe did not have a guard at the point of operation or over rotating parts. 3). A green Powermatic brand, model no. 1200, drill press did not have a guard at the point of operation (chuck).  Shipwright Shop at Hyde Street Pier A Delta Rockwell drill press did not have a guard at the point of operation (chuck).   Small Crafts Workshop at Hyde Street Pier: 1). Clausing lathe did not have any machine guarding over the in-going nip points of a rotating belt nor at the point of operation. 2). Powermatic drill press serial no. 11063454 did not have a guard at the point of operation (chuck).   Employees are exposed to potential contact with rotating parts or flying objects, which can cause eye injuries, lacerations, or amputations.

Exhibits area wood shop: The non-working portions of the blade of a Delta bandsaw, model number 28-206, serial number 302648, were not fully enclosed or guarded. The blade portion next to the Start/Stop button was not fully enclosed. There was a gap in the enclosure at the front of the blade.   2). Hyde Street Pier, Shipwright Shop: The non-working portions of the blade of a Delta bandsaw, series number 15-665, serial number 1491601, were not fully enclosed or guarded. The blade portion next to the Start/Stop button was not fully enclosed. There was a gap in the enclosure at the front of the blade.   3). Small Crafts Area: The blade portion next to the Start/Stop button was not fully enclosed on a green Delta band saw. There was a gap in the enclosure at the front of the blade.   Employees are exposed to potential severe lacerations or amputation of fingertips.

DW 625, serial no. 069704, had a nick in the cord that exposed the internal components. Employees were exposed to potential contact with electricity which can cause shock.

29 CFR  1915.71(c)(7): The minimum dimensions and spacing of members of independent pole wood scaffolding were not in accordance with section 1915.118 Table E-1.   On the historical ship Eureka, 2x2 pieces of lumber were being used as center vertical supports for a shipwright scaffold. The 2x2s were stacked on top of other pieces of lumber including wedges on top of a sawhorse. The scaffold was at the top of the stairs of the Eureka and potentially unstable. Employees were exposed to a fall hazard.

29 CFR  1910.179(j)(2)(iv): Hoist chains were not inspected monthly with a certification record which included the date of the inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected:  Located at the Hyde Street Pier, hoist chains on the gantry crane outside the small crafts shop were not inspected monthly with a certification record that contained all required elements. The hoist chains included 3 ton magna type hoist and Dayton hand chain hoist model 1VW59.
