29 CFR  1910.176(b): Storage of material created a hazard. Bags, containers, bundles., etc, stored in tiers were not stacked, blocked, interlocked, or limited in height so that they were stable and secured against sliding or collapsing.  Located at the San Leandro warehouse in zone 24, storage of wood created a hazard. In front of the sign for zone 24 on a wheeled cart, large blocks of timber were unevenly stacked in excess of 6' high without being tied down or otherwise secured.

29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   At the San Leandro Warehouse, forklift attachments were used without prior written approval from the forklift manufacturer. Attachments included:  1). Star Industries Fork Mounted Lift N Tow Model Number 1390B 2). Fork Extensions  Attachment use can alter the forklift's capacity or load center. Employees are exposed to potential crushing injuries from machine tipover or falling loads.

At the San Leandro warehouse, a 3-inch Lift-All Nylon web sling, type EN2-803, had a tear that exposed the inner material. Employees were exposed to potential crushing injuries from dropped loads.

Employees are exposed to potential amputation hazards from contact with the saw blade.

29 CFR  1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:   At the San Leandro warehouse, portable fire extinguishers had not had annual maintenance checks done within the last year. Extinguishers throughout the building had not been inspected since September 16, 2022. One extinguisher in the fire rated hazardous material storage area had not had an annual maintenance since 2012.

29 CFR  1910.334(a)(2)(ii): Where there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made:  At the San Leandro warehouse, two pieces of equipment had evidence of defects or damage in the electrical cords that could expose employees to injuries.  1). The outer cord sleeve of a yellow Fostoria Industries tower work light was damaged, exposing the wiring underneath at the connection point of the cord to the outlet receptacle fixed to the light.  2). The plug on a second yellow Fostoria Industries tower work light had scorch marks on it.
