29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  a) At Lot #57: On 12/22/2023, the employer exposed employees to struck-by hazards, in that no head protection was provided to employees while a truck mounted crane was in operation setting roof trusses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction on Lot #57, Top plate of the 2nd floor: On 12/22/2023, the employer exposed employees to fall hazard of approximately 18-feet, 9-inches to the lower level, in that employees were not provided with a fall protection system while engaged in framing operations on the top plate of the second floor.  b) New residential construction on Lot #57, 2nd floor: On 12/22/2023, the employer exposed employees to fall hazard of approximately 10-feet 3-inches to the lower level, in that employees were not provided with a fall protection system while using the second floor as the walking/working surface.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Residential construction on Lot #57: On 12/22/2023, the employer exposed employees to fall hazards of approximately 10-feet, 3-inches to the lower level, in that the step ladder does not reach the next working level at least 3-ft past the 2nd floor decking to provide a safe means of access/egress to the next working level.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) Residential construction on Lot #57: On 12/22/2023, the employer exposed employees to electric shock hazards, in that an extension cord with a missing grounding pin was used. An inspection of these cords were not conducted at the beginning of the shift to check for damage.
