29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Fabrication shop. On or about January 3, 2024 and times prior, the employer exposed employees to caught-in hazards, in that the machine guard for the Sesco Superiority (Model#5115, Serial# B464493) Beloit Angle Roller was not installed on the backside of the machine exposing employees to caught-in/ pinch points hazards while the employees are operating the machine.

On or about January 3, 2024 and times prior, the employer exposed employees to caught-in hazards, in that the machine guard for the Sesco Superiority (Model#5115, Serial# B464493) Beloit Angle Roller was ineffective where the guard was not secured on one side exposing employees to caught-in/ pinch points while the employees are operating the machine.
