29 CFR 1910.147(f)(1):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: a. The Ohio Art Co. - Bryan, Ohio: On or about December 22, 2023, employees were exposed to being caught in the ingoing nip point created between the C6 Coater metering rollers, located at the infeed side of the coater, when positioning and cleaning the rollers. The employer did not develop and utilize a procedure following a sequence of actions which removed employees from the hazard and isolate the machine from energy source(s) in accordance with paragraph (d) of this section or alternatively provide protection with effective alternative safeguarding measures during roller cleaning tasks. b. The Ohio Art Co. - Bryan, Ohio: On or about December 22, 2023, employees were exposed to the pinch point created between the C6 Coater application roller and an adjacent stationary metal bar, located at the infeed side of the coater, when performing positioning and cleaning tasks. The employer did not develop and utilize a procedure following a sequence of actions which removed employees from the hazard and isolate the machine from energy source(s) in accordance with paragraph (d) of this section or alternatively provide protection with effective alternative safeguarding measures during roller cleaning tasks. c. The Ohio Art Co. - Bryan, Ohio: On or about March 5, 2024, employees were exposed to the pinch point created between the C6 Coater blanket cylinder (red rubber roller) and the stationary nip guard, located at the outfeed side of the coater, when performing positioning and cleaning tasks. The employer did not develop and utilize a procedure following a sequence of actions which removed employees from the hazard and isolate the machine from energy source(s) in accordance with paragraph (d) of this section or alternatively provide protection with effective alternative safeguarding measures during roller cleaning tasks. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: a. The Ohio Art Co. - Bryan, Ohio: On or about January 3, 2024, employees were exposed to injuries associated with being caught in rotating power-transmission shafting located on the C6 Coater line. The horizontal shafting, used to convey metal sheets from the coater to the oven, was not adequately guarded. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a. The Ohio Art Co. - Bryan, Ohio: On or about December 22, 2023, the employer did not perform an adequate periodic inspection of the energy control procedures to ensure that lockout/tagout procedure and requirements of 29 CFR 1910.147 were being followed. The employer did not ensure that energy control procedures for Line #6 Coater (C6) were inspected while authorized employees performed servicing and/or maintenance activities. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
