29 CFR 1910.243(d)(4)(xii): Tools shall not be used in an explosive or flammable atmosphere: a) In the facility, on or about January 6, 2024 - The employer did not ensure that tools were not used in an explosive or flammable atmosphere. On January 6, 2024, an employee used a DeWalt Model DW888 2-inch Die Grinder to perform work on a vessel that had previously contained flammable gas. No measures were taken to purge the vessel or perform an atmospheric check for gas residue resulting in a fire/explosion.
