29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to crushing and caught-in hazards while operating an unguarded Upender Machine.  The Upender machine was not equipped with a guarding mechanism to prevent the platforms from coming down while employees were in the danger zone.    b) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to caught in-hazards from rotating parts and struck-by hazards from flying chips while operating an unguarded JET Milling/Drilling Machine.  c) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to laceration and amputation hazards while operating a JET 16" Horizontal Bandsaw that was not properly guarded.  The adjustable guard on the band saw was not adjusted to minimize employee exposure to the unused section of the saw.  Approximately four inches of the unused section of the blade was unguarded.

Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to electrical shock and fire hazards while working near a damaged power cord that powered a band saw in the maintenance department.

29 CFR 1910.303(g)(1)(i):The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:  a) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, the employer did not ensure employees were protected from electrical and fire hazards in that, clearance around disconnect knife switch boxes were less than that specified in Table S-1. Clearance space to Panel L3, Breaker 33, was impeded by a table placed directly in front of the electrical equipment.  This condition exposed employees to the hazard of electrical shock and burns.   b)  K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, the employer did not ensure employees were protected from electrical and fire hazards in that, clearance around disconnect knife switch boxes were less than that specified in Table S-1. Clearance space to Panel LP, Breaker 21, was impeded by a pallet loaded with material placed directly in front of the electrical equipment.  This condition exposed employees to the hazard of electrical shock and burns.

Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to electrical shock hazards while working around an uncovered electrical junction box in the wash tank area.
