29 CFR 1910.23(b)(8): The employer must ensure ladders are used only for the purposes for which they were designed. a. On or about January 4, 2024, located on the roof of the facility at the steam valve, the employer did not ensure that the employees were protected from falls while using a modified 14' fiberglass A-frame step ladder to access an elevated maintenance platform. The rear side of the designed self-supporting ladder was removed and the ladder was leaned against the piping as a non-self-supporting ladder.

On or about January 23, 2024, located on the roof of the facility at the steam valve, the employer did not ensure that a modified step ladder used by employees had been inspected, tagged, and removed from service due to unsafe conditions.
