On or about January 4, 2024, located on the roof of the facility at the steam valve, the employer did not ensure that the employees were protected from falls while using a modified 14' fiberglass A-frame step ladder to access an elevated maintenance platform. The rear side of the designed self-supporting ladder was removed and the ladder was leaned against the piping as a non-self-supporting ladder.
