29 CFR 1910.178(n)(1): All traffic regulations were not observed, including authorized plant speed limits. A safe distance was not maintained approximately three truck lengths from the truck ahead, and the truck was not kept under control at all times. On or about 12/30/2025, Warehouse: An employee was operating a Hyster Forklift, Model #E65XN-40 (FL 803) and did not ensure a safe distance of three truck lengths from the Automated Guided Vehicle (AGV) in front of them.

29 CFR 1910.178(n)(3): Other trucks traveling in the same direction at intersections, blind spots, or other dangerous locations were passed. On or about 12/30/2025: Warehouse: An employee was operating a Hyster Forklift, Model #E65XN-40 (FL 803) when they passed an Automated Guided Vehicle (AGV) caring a pallet in a dangerous location. The employee's forklift made contact with two parked forklifts causing the employee to suffer four amputated fingers.
