29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Production Floor: On or about 2/2/24, the employer did not ensure that the Rotomat was properly guarded. The light curtains that were mounted around the parameter of the Rotomat were inoperable and the electrical connections were not connected. b) Production Floor: On or about 2/2/24, the employer did not ensure that the Brother TC-32A, CNC Tapping Center, SER. No. 111138, was properly guarded. The door safety interlocks were bypassed. c) Production Floor: On or about 2/2/24, the employer did not ensure that the Brother TC-32A, CNC Tapping Center, SER. No. 111222, was properly guarded. The door safety interlocks were bypassed. d) Production Floor: On or about 2/2/24, the employer did not ensure that the Kitamura MYCENTER 4XV, SERIAL NO. 39344, was properly guarded. The door safety interlocks were bypassed.
