The light curtains that were mounted around the parameter of the Rotomat were inoperable and the electrical connections were not connected.  b) Production Floor: On or about 2/2/24, the employer did not ensure that the Brother TC-32A, CNC Tapping Center, SER. No. 111138, was properly guarded. The door safety interlocks were bypassed.  c) Production Floor: On or about 2/2/24, the employer did not ensure that the Brother TC-32A, CNC Tapping Center, SER. No. 111222, was properly guarded. The door safety interlocks were bypassed.    d) Production Floor: On or about 2/2/24, the employer did not ensure that the Kitamura MYCENTER 4XV, SERIAL NO. 39344, was properly guarded. The door safety interlocks were bypassed.
