Section 5(a)(l) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, including severe burns, to employees in that employees were exposed to fire, flash fire, deflagration, or explosion hazards from an improperly maintained dust collection system and duct work for combustible metal materials located inside a building : (a) In the Polishing Area - On or about June 18, 2024: (1) Accumulations of metal dusts were not removed promptly from horizontal surfaces adjacent to the dust collection systems including electrical conduit and boxes on walls and equipment. (2) The dust collection system's nonmetallic dust collection duct work was not properly grounded.

(a) In the Polishing Area: On or about June 18, 2024, the hood on the left-hand side pedestal polishing unit did not completely enclose the sides of the polishing wheel, exposing employees to the rotating shaft of the polishing unit.
