29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.  a. The employer does not ensure employees on a walking/working surface with an unprotected side or edge 6 feet (1.8 m) or more above a lower level are protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  This violation was discovered on or about January 29, 2024; on the second-floor front roof: where employees were painting and caulking holes on the outside of the second-floor walls and were exposed to fall hazards.  b. The employer does not ensure employees on a walking/working surface with an unprotected side or edge 6 feet (1.8 m) or more above a lower level are protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  This violation was discovered on or about January 29, 2024; on the platform above the second-floor stairway: where employees were painting, caulking and installing a wooden decorative design on the ceiling and were exposed to fall hazards.  c. The employer does not ensure employees on a walking/working surface with an unprotected side or edge 6 feet (1.8 m) or more above a lower level are protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation was discovered on or about January 29, 2024; on the second floor near the stairway: where employees working on the second floor were exposed to fall hazards.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the requirement, including describing the steps that it is taking to ensure employees on a on a walking/working surface with an unprotected side or edge 6 feet (1.8 m) or more above a lower level are protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76cm) whichever is less were not equipped with one stairrail system along each unprotected side or edge:   The stairway used to access the second floor was not equipped with a stair rail along the unprotected edge. Employees were exposed to fall hazards.   Nana's Construction Inc. was previously cited for a repeat violation of this Occupational Safety and Health Standard 29 CFR 1926.1052(c)(1)(ii), which was contained in OSHA inspection number 1644641, citation number 1, item number 4 and was affirmed as a final order on May 3, 2023, with respect to a workplace located at the Samoa Court House in Fagatogo, American Samoa.
