29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not designed for hard or extra-hard usage: In Front of Building: Single wires were used to power a light in the room, exposing employees to electrical hazards. Nana's Construction Inc. was previously cited for a repeat violation of this Occupational Safety and Health Standard 29 CFR 1926.405(a)(2)(ii)(J), which was contained in OSHA inspection number 1602832, citation number 1, item number 2 and was affirmed as a final order on October 26, 26 2022 with respect to a workplace located at the Fagatogo Court House, Pago Pago, American Samoa.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: West room: Flexible cords used to power the lights were routed through the doorway, and was pinched every time the door was closed and locked, exposing employees to electrical and fire hazards.
