29 CFR  1910.1053(f)(2)(i): The employer did not establish and implement a written exposure control plan that contained at least the following elements: a description of the tasks in the workplace that involve exposure to respirable crystalline silica; a description of the engineering controls, work practices, and respiratory protection used to limit employee exposure to respirable crystalline silica for each task; and a description of the housekeeping measures used to limit employee exposure to respirable crystalline silica:  a)	In the facility, on or about April 17, 2024 - The employer did not establish and implement a written exposure control plan to determine which tasks in the workplace involve exposure to respirable crystalline silica and which engineering controls and work practices the employer planned to implement to limit employee exposure during each listed task.

29 CFR  1910.1053(i)(1)(i): The employer did not make medical surveillance available at no cost to each employee, and at a reasonable time and place, for each employee who has been occupationally exposed to respirable crystalline silica at or above the action level for 30 or more days per year:  a)  At the facility, on or about April 17, 2024 - Medical surveillance were not made available to employees, who were exposed to respirable crystalline silica, at or above the action level, for 30 or more days per year.  An employee designated as a CNC/Polisher was exposed to crystalline silica at an 8-hour time-weighted average of 31.9 ug/m3.  This is approximately 1.27 times the action level of 25 ug/m3.  Sampling was performed by an OSHA compliance officer on April 17, 2024 for 401 minutes and zero exposure was assumed for the unsampled portion of the shift.  b)  In the Facility, on or about April 17, 2024 - An employee designated as a Polisher was exposed to Mixed Respirable Crystalline Silica at a time weighted average exposure of 56 micrograms/m3.  This level is 2.24 times the OSHA permissible exposure limit of 50 micrograms/m3 as a time weighted average concentration.  Sampling was performed by an OSHA compliance officer on April 17, 2024 for 398 minutes and zero exposure was assumed for the unsampled portion of the shift.
