29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 13, 2024, employees engaged in seed mill operations at the Central Valley Ag facility located at 102 South Main Street, Ainsworth, Nebraska, were exposed to caught in and amputation hazards in that the south robot conveyor access area did not have guarding to prevent entry into the area while the robot was operating.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven feet or less from floor or working platform were not protected by stationary casing enclosing shafting completely or by troughs enclosing sides and top or sides and bottom of shafting: On or about February 13, 2024, employees engaged in seed mill operations at the Central Valley Ag facility located at 102 South Main Street, Ainsworth, Nebraska, were exposed to an exposed rotating shaft for the West Mill Reclaim Drag in the grain silo basement. The shaft had bolt head protrusions, and there was a 3.5 inch exposed opening from either side.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: On or about February 13, 2024, employees engaged in grain and animal feed processing at the CVA facility located at 102 S Main St, Ainsworth, Nebraska, were exposed to electrical shock hazards in that temporary wiring was used to power a sewing machine. The temporary wiring was being used in lieu of permanent wiring for 1 year.
