All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about February 13, 2024, at a workplace located at 5757 West Custer Street, Manitowoc, Wisconsin in the Secondary Operations Department; employees did not physically locate or operate energy isolation devices for electrical and/or pneumatic pressure on the Niederberger Belt Sand machines when conducting preventative maintenance tasks, such as belt changes.
