OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards while operating a powered overhead crane: a. Adler Pelzer Group, 1150 N Wynn Rd, Oregon, Ohio 43616: On or about February 14, 2024, the hook of the Caldwell 3 ton Spreader Beam used on the R&M overhead crane in the "Ford Area," did not maintain a functional safety latch mechanism, effectively closing the hooks' throat. Employees using the overhead crane with spreader beam in this condition were exposed to crushing injuries associated with displaced loads.

The employer did not ensure that when wet processes are used, drainage is maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. a. Adler Pelzer Group, 1150 N Wynn Rd, Oregon, Ohio 43616: On or about February 14, 2024, the employer did not ensure that the work floor, located in and around Trim Press area was maintained in so far as possible a clean condition. Oil leaking onto the floor from the Trim Press exposed employees to slip and fall hazard. b. Adler Pelzer Group, 1150 N Wynn Rd, Oregon, Ohio 43616: On or about February 14, 2024, the employer did not ensure that the work floor, located in and around Carpet Line water jet area was maintained in so far as possible a dry condition. water from the water jet exposed employees to slip and fall hazard.
