Pin setting area: On/before February 13, 2024, the employee(s) conducted mixing and cleaning tasks with Defense-C Lane Cleaner on a weekly basis. The employer did not ensure a 2.5-gallon container for the toxic Defense-C Lane Cleaner had a label with the identity, health and safety information attached to it. This condition potentially exposed employee(s) to eye and skin itchiness, redness, irritations, and/or dermatitis.

Bowling Alley: On/before February 13, 2024, the employees conducted cleaning and disinfecting tasks with chemicals in different areas of this facility on a weekly basis. The employer did not ensure that the Safety Data Sheets for this facility were maintained and updated for employees use. This condition potentially exposed employees to chemical hazard(s).
